Placement Invoicing Form

Artists must submit completed work using this form. All submissions must include the date of each planning/classroom session or workshop completed and GST/HST number if applicable.

Final lesson plans must be submitted to [email protected] and your teacher partner.

Payment will be made upon completion of the services assigned and the receipt of this completed form. To ensure payments are processed, please submit no later than 30 days after the completion of work.
